Emergency Medical Program

The Emergency Medical Program provides gas cards for out-of-county doctor appointments.

The qualifications are as follows:

• • • Limit of 3 out-of-county trips per year
    • Doctor appointment must be within a week of the request for a gas card
    • Proof of appointment at the time of visit
    • Proof of address (mail received within past 30 days)
    • Proof of income for the all-household members (for past 30 days)
    • Photo ID
